Registration Requirements:
Pregnant Women or Mothers with a newborn up to 3 months old that reside in Seminole County.

Seminole County Proof of Residency: Copy of Lease, Utility Bill, Photo ID with Current Picture or Voter’s Registration Card
Proof of Pregnancy: Dated Ultrasound, Medical Provider Confirmation or Birth Certificate (0-3 months)
